Payment Terms
Once approved, Premium Healthcare Providers (PHP) will bill you every 15th and 30th of each month. We expect payment after 15 days.
Termination
The contract should be in effect upon receiving both parties’ signatures and shall continue until terminated by either party.
If termination does occur, our agency will continue to provide services through the last day of the month and notices must be delivered at least 14 days prior to the intended termination date.
Limitation of Liability
Our agency shall not be held liable for any damages resulting from the client’s use of our services and solutions unless our company has failed to act in knowing violation of the law.
Modification
All modifications to the agreement must be provided in writing and signed by both parties to be considered legally binding.
Warranties
Should the client determine that services were not rendered according to the agreement, it may provide written notice to our agency. Our agency shall re-perform or refund such services if they do not conform to the proposal agreed upon by both parties at no cost to our clients.
Severability
Should any provision of this agreement be found to be illegal or unenforceable, that provision is severed from this agreement and is to be replaced with a provision that is substantially similar while in accordance with governing law.
